Diagnostic probability of cell block preparations in vitreoretinal lymphoma
Purpose
The aim of this study is to examine the diagnostic probability using cell block (CB) methods between the laboratory tests.
Methods
This study enrolled 39 eyes in 34 VRL patients, and 7 eyes in 7 patients with idiopathic uveitis due to VRL-like ocular findings, who underwent diagnostic vitrectomy in Hokkaido University Hospital. Medical records including the results of CB cytology, interleukin (IL)-10/-6 ratio, and immunoglobulin heavy chain gene (IgH) rearrangement monoclonality in the vitreous were retrospectively searched. Diagnosis of VRL was made based on clinicopathological findings.
Results
Patients with VRL composed 17 women and 17 men, and the age of onset ranged from 44 to 86 years (mean: 70 years). CB preparations detected large malignant cells in 35 eyes (89.7%), whereas the other 4 VRL eyes were negative. Three out of the latter 4 eyes showed subretinal infiltrates in the preoperative fundus. Intravitreal IL-10 and -6 concentrations were 1,836±4,039 and 98±137 pg/ml, respectively, in which the positive rates being IL-10/-6 >1 was 87.2%. The presence of IgH monoclonality was 64.1%. In patients with uveitis, CB specimens revealed small inflammatory cells. IL-6 concentrations were 311.1±240 pg/ml, whereas IL-10 values were undetectable in 6 eyes. No eyes with uveitis showed IgH monoclonality.
Conclusion
The diagnostic probability of CB preparations in VRL was similar with that of IL and IgH. It is likely that VRL cases having subretinal infiltrates might deteriorate the diagnostic probability of CB preparations.
