Presenting Characteristics, Clinical Outcomes and Management of Acute Retinal Necrosis
Purpose
To evaluate the clinical, demographic characteristics, treatment approach and visual results of patients with acute retinal necrosis (ARN).
Methods
Demographic data, ophthalmological examination, etiologies, treatments, and complications of patients, who were followed up and treated with the diagnosis of ARN between 2000 and 2020, were evaluated retrospectively.
Results
24 eyes of 24 patients were included. The mean age was 43,7 years and mean follow-up period was 31 months. Polymerase chain reaction (PCR) test from eye fluid samples detected HSV in 7 eyes, VZV in 6 eyes, CMV in 1 eye, and Adenovirus in 1 eye. Central retinal occlusive vasculitis was observed in 3 eyes. In initial treatment, intravenous antiviral was used in 21 patients, oral antiviral was used in 3 patients, additionally intravitreal ganciclovir was administered to 7 eyes. The most common complication was retinal detachment (RD). Prophylactic laser was performed in 9 eyes, vitrectomy and laser photocoagulation (LPC) in 5 eyes. Vitreoretinal surgery was performed in 10 eyes for RD. RD development rates were similar in eyes with prophylactic LPC and vitrectomy + LPC and eyes without LPC (p=0.237). At the final examination, best corrected visual acuity was below 20/200 in 15 eyes. Fellow eye involvement was not observed in any cases.
Conclusion
ARN is an uveitic entity that is among eye emergencies. Clinical findings are essential in diagnosis, but PCR test from eye fluids is useful to support diagnosis. The role of prophylactic laser or vitrectomy is still unclear. RD is the most common complication.
