Ocular Toxoplasmosis: Is there a correlation between clinical features and complications with patients age?
Purpose
How does ocular toxoplasmosis (OT) manifest demographically and clinically? Is there a correlation between clinical presentation and complications with patients age?
Methods
Monocentric, retrospective observational cohort study. We reviewed the medical records of 290 patients with active OT. Various variables were analyzed and compared, including age at onset and recurrence, laboratory findings, therapeutic regime, lesion size, location, inflammatory activity, visual acuity and complications.
Results
Mean age at first presentation with an active OT lesion was 37.7 years. The mean reported age at the time of first ocular manifestation was 28.7 y. and 36.1 y. for patients with recurrence. Primary OT were observed in 29.1% and a combination of old scars and active lesions in 70.9% of patients at first presentation to the ophthalmologist. Increased inflammatory activity, extensive retinal lesions and worse visual outcome were related to older patient age. Macular involvement correlated positively with complications and worse visual outcome. Complications were more frequently observed in patients with primary OT and were mostly accompanied by pan uveitis and longer duration of the disease. Bilateral involvement was predominantly found in patients with congenital toxoplasmosis, who also showed more macular scars at first presentation and a poorer visual outcome.
Conclusion
Our patient cohort showed a significant correlation between age and several clinically relevant parameters. In addition, the location of the lesion turned out to be a predictive factor for the occurrence of OT-associated complications.
