TitleEffectiveness of intravitreal anti-VEGF injection,intravitreal corticosteroid implant and systemic steroid for treatment of recurrent uveitic macular edema:Case Report
PurposeIntravitreal anti VEGF,corticosteroid implant and systemic steroids are used to treat macular edema.The study compared the effectiveness of each intervention with a patient who had recurrent uveitic macular edema
MethodsA 59-year-old female known diabetic presented to our clinic with blurring of vision of the right eye. On examination, visual acuity was 20/400 on the right eye and 20/25 on the left. Fluorescein angiography and OCT revealed macular edema on the right eye. Data from the macular scans before and after interventions (intravitreal anti-VEGF injection,intravitreal corticosteroid implant and systemic steroid) were analyzed. Changes in the central subfield, outer and inner macular thickness were considered as outcome measures.Progress in VA was descriptively compared
ResultsMean thickness of the outcome measures differed significantly between interventions (F(1.205, 9.642)=15.397, p<.05)). Specifically, mean thickness was reduced in the second and third interventions by an average of 50.56μm(p<.05) and 94.33μm(p<.05), respectively, against the baseline. Patient responded most positively to the third intervention with reduced central subfield thickness of 47% against the baseline and improvement of VA to 20/25 from 20/80 prior to administration
ConclusionSystemic steroid was the most effective intervention resulting in a significant reduction in retinal thickness and remission from uveitic macular edema leading to sustained improvement in VA. Further evaluation of this treatment especially post-intervention review for side effects and future case-control study methodology are recommended
