| Title | Cytokine patterns of aqueous humor in neovascular age-related macular degeneration eyes with onset of geographic atrophy under aflibercept treatment |

| Purpose | To investigate cytokine patterns of aqueous humor in neovascular age-related macular degeneration (nAMD) eyes with onset of geographic atrophy (GA) for 2-year aflibercept treatment (IVA). |
| Methods | Twenty-two eyes of 22 nAMD patients were enrolled (males / females; 15 / 7, average age ± SD; 67.7 ± 11.2 years old). The patients were given IVA three times every 4 weeks, thereafter were treated by pro re nata for 2 years. Undiluted aqueous humor samples were collected before first IVA (before IVA) and before third IVA (after IVA). Levels of IL-6, IL-7, IL-8, IL-12, IL-13, Eotaxin, IP-10, MCP-1, MIP-1α, MIP-1β and VEGF were measured by a multiplex bead array. Principal component analysis was performed. |
| Results | Seven patients (24.1%) developed GA for 2 years. In nAMD patients with GA (GA+), first principal component (PC1) before IVA accounted for 52.9% of all variability in the data, and cytokines with principal component loadings (PCLs) of over 0.5 were IL-7, MCP-1, IL-6, VEGF, IL-8, MIP-1β, IP-10 (0.57), IL-13 and IL-12. After IVA, PC1 accounted for 61.7%, and PCL of IP-10 decreased to be -0.13 while PCLs of the other cytokines were not mostly changed. In nAMD patients without GA (GA-), PCL of IP-10 before and after IVA were -0.13 and 0.67, respectively. PCL changes of IP-10 before and after IVA were completely opposite between GA+ and GA-. |
| Conclusion | Immune responses of antifibrosis might be weaken after IVA in nAMD eyes with onset of GA. |
