| Title | Supportive role of Anterior Segment Optical Coherence Tomography in Uveitis-Glaucoma-Hyphema Syndrome |
| Purpose | To analyse the clinical features of patients diagnosed with uveitis-glaucoma-hyphema (UGH) syndrome and to evaluate the supportive diagnostic role of anterior segment optical coherence tomography (AS-OCT). |
| Methods | Retrospective retrieval of data incorporating all new UGH syndrome cases evaluated at the Ocular Immunology Unit in Reggio Emilia (Italy) from January 2019 to December 2022. |
| Results | All the 22 patients were Caucasian; 12 (54.5%) were male. The mean age at symptom onset was 59 (range 7-86) years. The mean (± standard deviation) time between cataract surgery and symptom onset and between symptom onset and UGH syndrome diagnosis were 93.9 (±77,3) and 15.2 (±14.8) months, respectively. Fifty percent of the patients had been erroneously diagnosed with idiopathic anterior uveitis and 22.7% with herpetic anterior uveitis. The most frequent clinical features were iris atrophy (63.6%), anterior chamber inflammation (59.1%), ocular hypertension (54.5%), vitreous involvement (50%) and IOL subluxation (45.5%). The majority of patients (90.9%) presented with intraocular lens (IOL) located in the ciliary sulcus, and one-piece was the most common IOL type (72.7%). All patients underwent AS-OCT, which showed a disruption of the normal reflectivity of the posterior iris surface due to iris-IOL contact. |
| Conclusion | In pseudophakic subjects affected by recurrent intraocular inflammation, UGH syndrome should be considered in the differential diagnosis. In these cases, AS-OCT is a rapid, non-invasive way to confirm the diagnosis. |
