Title	Complications in Acute Retinal Necrosis

Purpose	To evaluate the complications in a cohort of acute retinal necrosis (ARN) patients treated at a tertiary care center in India
Methods	A retrospective observational study in patients diagnosed as ARN visiting a tertiary eye hospital between 2004 and 2022
Results	The study includes 144 eyes of 119 patients with ARN. The mean age of involvement was 35.5±15.7 years (range 7-75 years) and 81 (68%) of them were male. 25 patients had bilateral presentation with simultaneous involvement in 10 cases. Average latency period for other eye involvement was 5.85 years (range2-10 years).The mean visual acuity at presentation was 1.58 ± 1.29 and 1.89 ± 1.43 at final visit on log MAR. Cataract developed in 114 (79.2%) eyes and 54.4% of them underwent cataract surgery. Retinal detachment occurred in 103 (72.5%) eyes. Cystoid macular edema (CME) was noted in 23 (17.2%) eyes. Ocular hypertension was noted in 30 (21.6%) eyes and was controlled on medical management. Other complications included hypotony (48.6%) epiretinal membrane (17.4%), optic atrophy (25%) eyes. Forty-nine (49/70,70%) eyes developed hypotony following surgical intervention, and twenty-one eyes had hypotony even without any intervention. The median follow-up duration was 735 days (60 days to 17.5 years). Recurrence of ARN noted in 6 eyes.
Conclusion	ARN has a guarded visual prognosis. Retinal detachment is the most common complication followed by hypotony and optic atrophy.
