| Purpose | To report four cases of scleritis/sclerokeratitis in association with hearing loss. |
| Methods | We report four cases of patients diagnosed with scleritis or scleroqueratitis and hypoacusis at our institution. Ophthalmic and torinolaringological studies ar shown. |
| Results | Case 1. Male 26 yo. Bilateral anterior scleritis. Severe neurosensory bilateral hypoacusis. Laboratory unremarkable. Acute blateral labyrinthitis in angioresonance. Interpreted as atipic Cogan syndrome. Meprednisone + Metotrexate. Good ocular response. Received bilateral coclear implant.
Case 2. Female, 60 yo, OS sclerokeratitis. Severe neurosensory bilateral hypoacusis. Positive ANCA C and anti PR3 antibody. Granulomatosis with polieangiitis diagnosis. Meprednisone + Azatioprin and switch to Metotrexate. Good ocular and hearing response.
Case 3. Female,15 yo, bilateral anterior scleritis. Bilateral hypoacusis and vestibular function abolition. Laboratory and imaging screening unremarkable. Interpreted as atipic Cogan syndrome. Meprednisone + Mycophenolate Mofetil. Good ocular and hearing response.
Case 4. Female, 6 yo, liver transplant history (bile duct atresia). OD sclerokeratitis, OS anterior scleritis. Hipoacusis and dizziness. Positive ANCA P, MPO. Other organs not involved. Topical prednisolone + tacrolimus. Good ocular and hearing response. |
| Conclusion | This case series highlights the association between scleritis and hypoacusis in patients with autoimmune disorders. Early identification and management of both ocular and auditory manifestations are crucial for optimizing patient outcomes, specially to prevent long term hypoacusis since this compromise can lead to permanent hearing loss if is not treated early in the course of the disease |
