TitleA case of bacterial endophthalmitis and meningitis due to Streptococcus suis

PurposeWe report on a case of bacterial endophthalmitis, and meningitis caused by Streptococcus suis.
MethodsA case report.
ResultsA 49-year-old man, a chef handling pork, suddenly developed fever, impaired consciousness, neck stiffness and bilateral hearing impairment, He had previously suffered from uncontrolled diabetes (9.0% HbA1c). Because blood cultures were positive for Streptococcus suis, he was diagnosed with bacterial meningitis and intravenous antibiotics started. The next day, he suffered from a decreased vision in the left eye. The visual acuity of the left eye was light-perception, Anterior chamber cells 3+ with fibrin were observed, but no hypopyon. The fundus could not be observed due to fibrin deposition. Bacterial endophthalmitis was suspected, and pars plana vitrectomy with cataract surgery were performed. During surgery, vitreous abscess, extensive retinal exudation and necrosis were observed. The eye was irrigated with a perfusion solution containing antibiotics, and silicone oil was injected at the end of the surgery. Streptococcus suis was also detected in the vitreous fluid. Postoperative antibiotic treatment eliminated intraocular inflammation, and the final best corrected-visual acuity was 0.09. Bilateral sensorineural hearing loss was diagnosed with labyrinthitis with bacterial meningitis.
ConclusionStreptococcus suis infection is more likely to occur in people who work with pork. The infection frequently causes meningitis and hearing loss and can rarely cause endogenous bacterial endophthalmitis. When searching for the cause of endophthalmitis, occupational history and general symptoms may be important. Pars plana vitrectomy is useful for definitive diagnosis and treatment.
