| Title | Retinal Vasculitis: A 30-Year Follow-Up Study in a Tuberculosis Endemic Country |

| Purpose | To describe clinical course of patients with retinal vasculitis (RV) with a follow-up for three decades. |
| Methods | Retrospective chart review of patients with RV with minimum 30 years of follow-up in a tertiary eyecare institute in India. |
| Results | 62 eyes of 37 patients were included in this study. The mean age of the patients at presentation was 31 ± 15.6 years, and 97% of them were male. In 54% of the patients, the involvement was bilateral. Five patients who initially presented with unilateral involvement subsequently developed involvement in the other eye. An association with presumed tuberculosis (16.2%) was found in six patients, and in the rest of the cases, the causes were unidentifiable. None of the patients devlope any clinical manifestations of ocular tuberculosis during this long follow-up period. The use of immunosuppressives was low (3 patients), as was the use of intravitreal anti-VEGF (1 patient). The primary modalities of treatment were oral corticosteroids and photocoagulation. The most common complications were vitreous hemorrhage (48.4%), fibrovascular proliferation (5.5%), glaucoma (22%), and retinal detachment (8.1%). Recurrence of inflammation was seen in 18 (30.6%) eyes. Nineteen (32.2%) eyes required vitrectomy, and 5 (8.5%) eyes underwent scleral buckling for retinal detachment. The mean follow-up duration was 34.3 ± 2.9 years (Range: 30-44 years). Thirty years later, it was seen that 41.9% of eyes had visual improvement, 24.2% of eyes remained visually stable, and vision deteriorated in 33.9% of eyes, of which 4.8% ended up having no perception of light. |
| Conclusion | Idiopathic RV remains a clinical entity with a challenging visual prognosis and a variable course and outcome. |
