TitlePerformance of the ISOLD score in the diagnosis of vitreoretinal lymphoma

PurposeThe Interleukin Score for Intraocular Lymphoma Diagnosis (ISOLD) was developed to calculate the probability of vitreoretinal lymphoma, from concentrations of interleukin (IL) 6 and 10 in the aqueous humor (AH) or vitreous. Our aim was to evaluate the effectiveness of the ISOLD score for LVR diagnosis
MethodsA single-center retrospective observational study includes all patients who had an eye sample (AH or vitreous tap) with dosage of IL-6 and IL-10 and calculation of the ISOLD score from 2017 to 2019. The first endpoint evaluated the sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of the ISOLD score. The secondary endpoint evaluated the effectiveness of the IL-10/IL-6 ratio for the diagnosis of LVR.
ResultsAmong 167 patients, 200 samples were analyzed with 171 anterior chamber taps and 29 vitrectomies. The sensitivity, specificity, PPV and NPV of the ISOLD score were 87%, 99%, 95% and 96%, respectively. Among the 6 false negative samples, 3 were taken while systemic corticosteroid or chemotherapy and one immunocompromised patient. The sensitivity, specificity, PPV and NPV of the IL10/IL-6 ratio for a threshold of 0.6 in aqueous humor were 89%, 94%, 75% and 98% respectively; and for a threshold of 1 in the vitreous 85%, 100%, 100% and 75% respectively.
ConclusionThe ISOLD score is an effective and reliable tool for the diagnosis of diffuse large cell B-LVR. Its interpretation must be done in addition of the values of the IL-10 and IL-6 concentrations and the IL-10/IL-6 ratio after ensuring that there is no ongoing treatment with chemotherapy or corticosteroid therapy to limit the risk of false negatives.
