TitleSankara Nethralaya Tubercular Uveitis (SANTU) Study- A Prospective Study
PurposeTo report the clinical spectrum of uveitis in patients diagnosed as presumed tubercular uveitis from a tertiary eye care centre in South India.
MethodsPatients with clinical features and laboratory investigations suggestive of ocular tuberculosis were enrolled prospectively between May 2018 and September 2022. All the patients underwent a complete ophthalmic and necessary imaging tests along with laboratory investigations. All patients received anti-tubercular treatment (ATT) for nine months under the guidance of a pulmonologist.
ResultsA total of 145 eyes of 93 patients with the diagnosis of ocular tuberculosis were included. The mean age of these patients was 36± 12 years (10 to 63 years), and 59 (63%) patients were males. A total of 52 (56%) patients had bilateral involvement. Intermediate uveitis (30%) and posterior uveitis (30%) were the most common clinical presentation, followed by retinal vasculitis (28%), panuveitis (8%) and anterior uveitis (5%). The majority of the patients (68%) were treated with the addition of corticosteroids with ATT. Further, immunosuppressive agents were used in 30 (32%) patients. Cystoid macular edema (34 eyes, 23%) was the most common complication. Azathioprine (67%) was the most common used immunosuppressive agent. The mean BCVA at the initial visit was 0.43 ± 0.65 log units. After a mean follow-up duration of 27 ± 14 months, the final BCVA was 0.24 ± 0.56 log units. Forty-three of 145 (30%) eyes had improved visual acuity, 9 (6%) eyes worsened, and 93 (64%) eyes remained stable.
ConclusionIntermediate and posterior uveitis are the most common presentation of intraocular tuberculosis in South India. ATT with steroids and immunosuppressive agents can improve vision.
