| Title | The Rescue Effect of Dexamethasone Intravitreal Implant in the Treatment of Recurrent Macular Edema after Cataract Surgery in a Case of Chronic Vogt-Koyanagi-Harada Disease - Case Report |
| Purpose | We described the rescue effect of Ozurdex in the treatment of recurrent macular edema (ME) after cataract surgery in a case of chronic Vogt-Koyanagi-Harada (VKH) disease. |
| Methods | Interventional case report. |
| Results | A 66-year-old male was referred for uveitis OU in 2018. His initial BCVA was 20/60 OU. Panuveitis besides disc edema were noted and OCT revealed subretinal fluid. Oral prednisolone and topical steroids were prescribed. ME persisted. H lost followed up for 5 months. In 2020, his BCVA deteriorated to 20/200 and 20/1000. Posterior synechiae, dense cataract, and sunset glow fundus were noted. OD OCT revealed ME. He received 3 posterior subtenon triamcinolone injections OU and cyclosporine 200mg daily. His ME subsided for 3 months. Subsequent OU cataract surgery were done. ME still recurred OD one month later even under cyclosporine 200 mg daily regimen. Ozurdex was implanted and ME subsided. Cyclosporine began to taper. Currently, only topical steroid and cyclosporine 25 mg daily were prescribed. His BCVA improved to 20/25 and 20/22. |
| Conclusion | Ozurdex was effective as the rescue therapy. Ozurdex appear to lessen the dosage of systemic immunosuppressants required for controlling recurrent ME. |
