TitleAtypical presentation of cytomegaloviral retinitis in an immuno-compromised patient
Purpose To present the atypical presentation of cytomegaloviral (CMV) retinitis in an immuno-compromised patient
MethodsWe report a case of CMV retinitis with atypical feature. The disease course, lab examination result, medical treatment, and response were presented.
ResultsA 34 year-old-lady was under immunosuppressant treatment for SLE. She had 1. Peripheral retinal necrosis OU. 2. Large area of retinal hemorrhage over necrotic retina. 3. Arterial obstructing vasculitis and venal sheathing over necrotic retina. 4 Distant arterial obstructing vasculitis. There was no vitreous inflammation. CMV retinitis was impressed, but classic CMV retinitis is more often at posterior pole along vascular arcade. Also distant obstructing arteritis is uncommon in CMV retinitis. Aqueous – PCR proved the diagnosis of CMV retinitis.
ConclusionCMV retinitis may present differently according to host immunity. Partially immunocompromised hosts may have peripheral granular necrotizing retinitis, vitritis, and severe occlusive vasculopathy.
