TitleBILATERAL ANTERIOR AND POSTERIOR HYPOPYON IN A PATIENT WITH SYPHILITIC PANUVEITIS
PurposeThis case highlights an atypical presentation of ocular syphilis, which to the best of our knowledge has not been reported before.
MethodsHistory and clinical examination, fluorescein angiography (FFA), optical coherence tomography (OCT), and magnetic resonance imaging (MRI) brain
ResultsWe report a case of 55 years Asian male with bilateral progressive visual loss with redness and photophobia. Ophthalmic examination revealed presence of bilateral anterior hypopyon, vitritis, posterior hypopyon as well as retinitis. Physical examination was suggestive of bilateral palmar rash and perianal condyloma lata. Patient was positive for serum venereal disease research laboratory (VDRL) test and human immunodeficiency virus (HIV) ELISA test. He was diagnosed and managed as ocular syphilis and resulting complete resolution of ocular inflammation.

ConclusionSyphilitic uveitis can have protean clinical manifestations and high index of suspicion is necessary in order to avoid misdiagnosis. It’s important to rule out infectious causes of uveitis before starting the patient on steroids. It’s important to screen for Human Immunodeficiency Virus (HIV) in all patients of syphilis presenting with ocular involvement.
