TitleCases of rhegmatogenous retinal detachment masquerading as panuveitis
PurposeTo present two cases of rhegmatogenous retinal detachment which were initially presented with anterior chamber inflammation and retinal detachment, masquerading as panuveitis.
MethodsA 64-year old female was referred for unilateral persistent panuveitis with serous retinal detachment in spite of oral corticosteroid therapy for several weeks. She was pseudophakic, had a past history of uveitis in the same eye one year ago. Slit lamp examination revealed anterior segment inflammation with cells (grade 3+) and retinal detachment in the right eye. Another case was a 27-year old female who presented with refractory uveitis with secondary glaucoma in the right eye. She also presented anterior chamber cells (grade 4+), fine keratin precipitates, and retinal detachment without definitely visible retinal break.
ResultsThese patients showed persistent retinal detachment in spite of oral corticosteroid therapy. Under the diagnosis of concurrent anterior uveitis and rhegmatogenous retinal detachment, surgery was performed in both patients. Retinal detachments were completely resolved following surgery, and retinal breaks were identified at far periphery during vitrectomy. Anterior segment inflammation was controlled with intensive topical treatment postoperatively.
ConclusionThe presence of significant anterior segment inflammation may masquerade rhegmatogenous retinal detachment as serous detachment associated with uveitis. The possibility of unidentified retinal breaks should be considered, especially in uveitic patients with pseudophakia, few vitreous cells, and retinal detachment that persists despite systemic corticosteroids use.
