TitleAdult ocular toxocariasis: intermediate uveitis and complication /Pattern of 3 intermediate uveitis in adult ocular toxocariasis.
PurposeTo describe the clinical features and the diagnosis of adult ocular toxocariasis
MethodsPatients between 2019 and 2021 at a referral eye care center with an intermediate uveitis with a diagnosis of ocular toxocariasis were included.
ResultsThree patients, 2 women and 1 man, with a mean age of 46 years were included. Patients presented an non granulomatosis unilateral intermediate uveitis with a chronic macular oedema, a granulomatous panuveitis or a retinal detachment with an intermediate uveitis. 3 patients presented some vitreous condensation. 1 patients had a chronic uveitis who didn’t respond to anti-inflammatory therapy and a chronic cortico-dependant macular oedema. 2 patients had a retinal detachment.
Diagnosis was established with an aqueous tap for 3 patients, on serology for 1 patient, and with a vitreous biopsy during a pars plana vitrectomy for 1 patient. Mean Baseline Visual Acuity was 20/120 and at 1 month was 20/50.
Treatment: 3 patients had antiparasitic treatment associated with an oral corticotherapy. 1 patient had intravenous bolus injections of solumedrol.
1 patient had pars plana vitrectomy (PPV) for retinal detachment. Inflammation was persistent at 3 month for 2 patients.
ConclusionAdult ocular toxocariasis is a rare pathology but we need to think about it, in front of an intermediate granulomatous uveitis. The treatment lays on antiparasitic medication and corticosteroids. Vitreous condensations, subretinal exudation and retinal detachment, are evocative signs.
